Nomination Form

WeConservePA is requesting nominations for Lifetime Conservation Leadership Award.

To nominate an individual, fill out the form below.  Please include  a one-page narrative explaining why this individual should be considered for this award. You may submit more than one nomination by submitting a separate form and narrative for each.

Nominator Expectations

By submitting your nomination you agree to assist WeConservePA by taking the following actions if your nominee is selected to receive the Lifetime Conservation Leadership:

• Gather and provide to WeConservePA a variety of photographs of the recipient representative of the span of their career.
• Obtain testimonials regarding the recipient from those who can comment on their work and character.
• Work with the award recipient to provide WeConservePA with an invitation list to presentation ceremony.
